MYSA Forms

Fill out the MYSA Guest Player Form out to use or loan a Guest Player from INSIDE or OUTSIDE of KCSG

Fill out the Missouri Permission to Secondary Roster Form for a player who wants to dual card with a team outside of KCSG

The Missouri Permission to Participate form must be completed and signed by all parties for acknowledgement and authorization of a youth player to participate in a competitive game OUTSIDE of their own club prior to the tryout period.

Uniform Information

Before a parent can order uniforms, the Team Manager needs to add a player to the team roster so the Soccer Master MyUniform system can send out Uniform invitations to parents to select sizes and pay for uniforms.
